A leading NHS hospital in London is seeking a clinical pharmacist to join their Post-Acute Medicine team. This role involves providing specialist clinical pharmacy services and working collaboratively with multidisciplinary teams to ensure effective patient care. The ideal candidate will be a registered pharmacist with a postgraduate qualification and significant experience in hospital pharmacy. Emphasis will be on clinical governance and quality improvement, ensuring safe and effective pharmaceutical care tailored to patient needs.
This post provides an excellent opportunity for an enthusiastic, dynamic and motivated pharmacist with a clinical pharmacy background to join the Post-Acute Medicine team.
The post holder will work as part of a dedicated pharmacy team to provide and develop specialist clinical pharmacy services within post‑acute medicine. The post holder will liaise between wards, departments and pharmacy to ensure that pharmacy services are tailored to meet the needs of the Care Group wherever possible.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
