A leading healthcare organization is hiring passionate Part-Time Personal Support Workers (PSW) for their Home Hemodialysis Program. The role involves providing dialysis-related care in patients' homes, ensuring seamless support after hospital discharge. Ideal candidates must hold a PSW Certificate, have excellent attendance records, and possess strong interpersonal skills. The position offers a sign-on bonus, competitive compensation, and a supportive work environment, allowing for flexibility in shift schedules.
The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home.
We are currently hiring passionate Part-Time Personal Support Workers (PSW) for our Home Hemodialysis Program, who want to be part of our Dialysis program. This new clinical model of care is an exciting opportunity for the right personal support worker and will allow a select group of patients to receive Renal Care within their community, such as a long-term care home or a client's home (home care).
The PSW will provide dialysis-related care activities in the patient's home. Registered Nurses from Regional Renal Program hospitals will teach and/or delegate dialysis-related care activities to PSWs and provide the necessary patient-specific training for each PSW. In this program, the PSW’s scope of work is limited to dialysis-related activities only
