Job description

At the heart of Greater Manchester's innovative Crisis Care Pathway, our unified service offers a comprehensive range of support for children and young people facing mental health challenges. Operating seamlessly across multiple pathways—including Rapid Response, Home Treatment, and support for discharges from medical wards—we provide timely, community‑based interventions aimed at preventing hospital admissions and facilitating safe, supported discharges.

As a key member of our team, you'll play a pivotal role in delivering high‑quality, compassionate care that makes a tangible difference in the lives of young people and their families. We are committed to fostering a supportive environment where every team member can thrive and contribute to the continuous development of our award‑winning services.

We welcome applications from registered nurses, social workers, and occupational therapists eager to make a meaningful impact. Whether you're looking to specialise in children's mental health or seeking a new challenge, we offer tailored training and development opportunities to support your growth within our multidisciplinary team.

Be part of a forward‑thinking service that is shaping the future of crisis care for young people. Apply now to join our dedicated team and help us continue to provide exceptional care across Greater Manchester.

You’ll be a core member of our Crisis Care team, delivering rapid, community‑based support through Rapid Response, Home Intensive Treatment, and medical discharge pathways—helping prevent hospital admissions, support safe returns home, and empower young people to stay in their own homes.

Key Responsibilities:
  • Assessment & Intervention: Conduct psycho‑social and risk assessments, create tailored care plans, and deliver creative interventions to support mental well‑being.
  • Multidisciplinary Collaboration: Work closely with CAMHS, acute and specialist hospitals, police, education, and social care to provide coordinated support.
  • Crisis Management & De‑escalation: Use advanced de‑escalation techniques to manage acute distress and reduce risk.
  • Community‑Based Support: Provide intensive home‑based care to prevent A&E visits and inpatient stays, boost family/community integration, and build resilience and life skills in CYP.
  • Shared Caseloads: Manage a small caseload within a multidisciplinary team, offering mutual support and ensuring comprehensive care delivery.
Operational Hours:

This role includes evening and weekend shifts (8am–10pm, 7 days a week).

If you bring strong assessment, communication, and crisis‑intervention skills and want to deliver compassionate, community‑centred mental health care, we’d love to hear from you. Join Manchester University NHS Foundation Trust (MFT) and become part of a 30,000 plus strong team, delivering exceptional care to over 1 million people every year.

As the largest provider of specialist services, and our Oxford Road Campus in Manchester, the largest health academic campus in Europe, we are forward‑thinking, committed to clinical excellence and driven to deliver innovation, and continuous improvement. Whether you’re beginning your career or taking the next step, MFT offers a stimulating and supportive environment where your contribution truly matters. Our Hive Electronic Patient Record system enables safer, smarter, and more efficient ways of working – empowering you to deliver the best possible outcomes for our patients.

You’ll be part of a collaborative, multi‑disciplinary community, working alongside professionals from a wide range of clinical backgrounds. Together, we push boundaries, share knowledge, and deliver care that transforms lives.

At MFT, you’ll find a workplace where everyone is respected, supported, and encouraged to thrive. We offer clear clinical pathways for professional development, helping you grow your skills and achieve your ambitions. Your journey at MFT is more than a job – it’s an opportunity to make a lasting impact. We’ll support you as you shape the future of healthcare, one patient at a time.
