A leading humanitarian organization in the UK is seeking a Senior Practitioner to coordinate referrals and mentor staff in delivering high-quality care. The role involves managing risk assessments, supporting individuals transitioning from hospital to home, and ensuring service continuity. Ideal candidates will have a SVQ Level 3 in Health and Social Care and experience in person-centred planning. - Applicants must have a full UK licence (2 years) and access to a vehicle.
