Overview

Job summary: A 12 month secondment to cover the Trust VTE Prevention Clinical Nurse specialist role. The VTE Prevention CNS leads on VTE prevention care, adherence to NICE VTE guidance, surveillance, analysis and improvement of VTE risk assessment rates, hospital associated thrombosis cases, VTE staff education, VTE patient information and appropriate use of thromboprophylaxis measures. Activity will involve data collection, analysis, presentation and dissemination, staff education activities, awareness initiatives and quality improvement work. As part of the thrombosis nursing team, the post holder will participate in regular clinical activity in the nurse led anticoagulation and DVT services.

Main duties and responsibilities

Clinical Responsibilities

  • To develop first hand understanding and familiarity of the VTE prevention clinical processes used across the Trust.
  • To develop expert knowledge around VTE prevention and thrombosis management.
  • To act as a resource person for the Trust around VTE prevention and thrombosis management.
  • To work autonomously as an expert practitioner in the nurse led anticoagulation and thrombosis services providing safe and effective patient centred care.
  • To attend MDT meetings, ensuring all patients are represented.
  • To participate in the identification and development of clinical protocols and strategies to enhance both the continuity and standard of specialist care whilst ensuring equity of access to the service.

Leadership

  • To accept responsibility for VTE prevention leadership locally and take initiative to seek performance surveillance and data collection to inform robust quality improvement work.
  • To use initiative and build strong working relationships to positively influence VTE prevention practice across the Trust.
  • To actively support clinical staff to provide high quality VTE prevention care.
  • To contribute positively to the leadership of nursing within the care group acting as an effective role model.
  • To actively involve service users in providing feedback of their experience of the current service and suggestions for improvement.

Job responsibilities

The Jobholder will carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of programmes of care. The jobholder will provide highly specialised nursing advice specific to the patient group or specialty. The job holder will contribute to the development of the specialty service as well as contributing to the strategic direction of nursing within the Trust. The jobholder will manage and lead a band 6 VTE nurse specialist and be responsible for the effective use of resources within the clinical team.

Principal duties and areas of responsibility

  • Acts in accordance with the Nursing and Midwifery Code of Conduct
  • Clinical care delivery: provide clinical leadership, deliver high quality care as an expert practitioner, undertake clinical procedures, maintain a safe working environment
  • Responsibilities for patient care: assess, plan, implement and evaluate specialist care using advanced clinical knowledge and skills; plan service provision; provide specialised advice
  • Freedom to act: make clinical assessments and treatment decisions within policies; refer to MDT; supervise and assess care outcomes within the team
  • Communication: maintain and ensure accurate records; support patients and families; liaise with Community Services for seamless care
  • Education and Training: provide and facilitate training for learners and junior staff; participate in personal development planning; participate in conferences; educate patients and carers
  • Leadership and management: provide leadership to the clinical team; line management of the specialist nursing team; ensure workload and performance data are collected and used for improvement; investigate incidents; contribute to nursing practice development and policy
  • Research and Audit: contribute to the research agenda; undertake audits; participate in clinical trials as appropriate
Qualifications and experience

  • Registered Nurse (Level 1)
