A healthcare provider in Norwich is seeking a Care Coordinator for maternity cover. This position involves supporting patients with complex needs, utilising health intelligence to deliver personalised care, and coordinating care for patients in care homes. Applicants must have experience in a GP Practice or Primary Care Network, and GCSEs in English and Maths are required. This role may lead to a permanent position but is not guaranteed.
#TeamCastle are looking for a fun and caring person to join their Care Coordinator team to support patients with complex or additional needs to access health and wellbeing support.
#TeamCastle currently have five Care Coordinators led by the Social Prescriber and Care Coordinator Lead who work alongside the wider Practice and PCN Team of clinicians and allied health professionals. We are looking for maternity cover for 12 months within this team in order to continue development into new areas to support ours and the Primary Care Network's patients. This position may lead to a permanent role but this can not be guaranteed.
The role is to cover a Monday and a Friday and applicants should only apply if they are able to work on these days.
