Overview

We are seeking an experienced and motivated Community Heart Failure Specialist Nurse to join our expanding service. This is a rewarding opportunity to make a significant difference to patients living with a confirmed diagnosis of Heart Failure—supporting them to manage their condition safely at home, reducing unplanned hospital admissions, and improving long-term quality of life.

Responsibilities
  • Provide expert clinical knowledge and specialist nursing skills in the management of Heart Failure across the community setting.
  • You will hold a defined caseload, delivering comprehensive assessment, treatment optimisation, education, and follow-up to both newly diagnosed and existing patients.
  • Working autonomously—yet fully supported by the wider MDT—you will:
    • Provide proactive management of patients to avoid unplanned hospital admission.
    • Optimise medication and treatment plans in line with national and local Heart Failure guidelines.
    • Deliver patient and carer education to support confidence and self-management.
    • Liaise closely with GPs, secondary care cardiology teams, community nursing, therapy services and social care to ensure seamless, coordinated care.
    • Advocate for high-quality Heart Failure care in line with NSF standards and contribute to ongoing service development and strategic planning.
    • Use advanced clinical skills and evidence-based knowledge to identify investigation and/or treatment needs of individuals.
    • Provide medicines management support and advice in accordance with national and local guidelines.
    • Provide appropriate advice and support to patients and their carers including the provision of education and the promotion of self-management skills.
    • To liaise with local cardiac rehabilitation teams and End of Life teams as appropriate.
    • Act as a specialist resource for patients, their carers and other health professionals to improve the management of patients with heart failure.
    • Liaise closely with primary and secondary care services to provide an integrated care plan in which the patient and carers participate.
    • Ensure mechanisms in place for escalation of care at times of clinical deterioration.
    • To triage referrals from GPs, consultants, inpatient heart failure team and other professionals and service in agreement with individual patient.
    • Provide empowerment and support, acting as an advocate to enable individuals and their carers to actively engage in agreed treatment programmes.

  • Manage and facilitate the care of patients with Heart Failure in the community through outpatient clinics and home visits.
  • Provide specialist assessment for individuals with heart failure and coordinate individual care plans to maximise health outcomes.
  • Use advanced clinical skills and evidence-based knowledge to identify investigation and/or treatment needs of individuals.
  • Provide medicines management support and advice in accordance with national and local guidelines.
  • Maintain and further develop expertise within the Heart Failure field and pursue continuing professional development.
  • To evaluate own knowledge, practice and learning needs as part of KSF and appraisal in professional development plans with line manager.
  • Demonstrate empathetic communication with all patients and respond to communication from patients and carers.
  • Provide up-to-date written and verbal communication on patients to other professionals in the hospital, community and across boundaries.
