What you will do:

• Domiciliary Visits: Conduct home visits to provide comprehensive medication management, including medicines reconciliation and immediate support for patients.
• Investigation of Missing Controlled Drugs: Ensure safety and compliance by investigating incidents related to controlled drugs. Implement preventive measures to enhance the security of medication handling.
• Huddle Participation: Engage in huddles to triage discharge letters, resolve medication safety issues, and improve medication optimisation efforts, contributing to the reduction of hospital readmissions.
• Medication Management: Collaborate with wider CCS teams and adult social care services to ensure effective medicines management, leveraging your expertise to optimize patient outcomes.

Requirements:

• Hold a BTEC or NVQ Level 3 in Pharmaceutical Services adequate for registration with the General Pharmaceutical Council.
• Possess significant experience in medicine management with a comprehensive understanding of its application in community settings.
• Have up-to-date knowledge of drug use and the issues surrounding stock control.
• Demonstrate an understanding of the importance of maintaining quality service monitoring and confidentiality.
• Post-qualification, show validated competence in dispensary services, experience working to and contributing to standard operating procedures, and providing customer-oriented services.
• Exhibit experience in patient medication counselling, working in multidisciplinary teams, and fostering medicine compliance and concordance.
