Responsibilities

  • Manage an allocated caseload within the Mental Health Wellbeing Team, providing care coordination and support.
  • Participate in ward reviews, placement reviews, and arrange and attend Care Programme Approach (CPA) reviews.
  • Conduct assessments of new referrals and liaise with inpatient services, community mental health services, and external agencies.
  • Cover Duty and contribute at Depot and Clozapine Clinics as part of the regular rota.
  • Actively participate in weekly zoning meetings and monthly business meetings, sharing updates and insights.

Job Summary:
We are offering an exciting locum opportunity for a Band 6 Community Psychiatric Nurse / Care Coordinator within the Havering Mental Health & Wellbeing Team – SOUTH JWN2, based at Oasis House, 28–30 Gubbins Lane, Harold Wood, RM3 0QA. This full-time role runs Monday to Friday, 09:00 to 17:00, with some flexibility. You will play a pivotal role in supporting the community’s mental health and wellbeing.
