Key Responsibilities
  • Conduct person-centred assessments under the Care Act 2014 to identify needs and develop effective support plans.
  • Promote independence using strength-based approaches, ensuring adults and carers can make informed decisions.
  • Collaborate closely with healthcare, housing, and community partners to provide integrated and seamless care.
  • Undertake safeguarding enquiries in accordance with Section 42, assessing and managing risks effectively.
  • Maintain high standards of record keeping, ensuring compliance with statutory and professional frameworks.
  • Engage fully in the ASYE programme, participating in supervision, reflective practice, and ongoing professional development.
  • Contribute to the continuous improvement of Adults Social Care services within Liverpool City Council.
About You
  • You are a compassionate, motivated, and reflective practitioner who is passionate about promoting social justice and empowering adults. You thrive in a collaborative environment and demonstrate resilience, empathy, and professionalism in every interaction.
  • Degree in Social Work or equivalent, recognised by Social Work England.
  • Registered or eligible for registration with Social Work England.
  • Eligibility to complete the ASYE programme.
  • Understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.
  • Strong communication, analytical, and organisational skills.
  • Commitment to equality, diversity, and anti-discriminatory practice.
  • Previous experience or placements in adult services, community care, hospital discharge, or safeguarding teams.
  • Familiarity with Liverpool’s local safeguarding frameworks and multi-agency working.
  • Confidence in operating within multi-disciplinary environments.
