A large integrated healthcare organization is seeking a support worker to assist in caring for patients with complex care needs. The candidate will work as part of a multi-disciplinary team, providing essential one-on-one care and support for daily living activities. Responsibilities include monitoring patient health, providing reassurance, and maintaining effective communication. This role is critical in enabling patients to achieve their personal goals and improving their quality of care. Suitable candidates will have strong interpersonal skills and the ability to work semi-autonomously.
The post holder will work as an integrated member of the multi-disciplinary team (MDT), working to support patients who present with complex and challenging enhanced care needs. The post holder will be required to use their initiative when working on the wards to implement person-centred plans of care and are expected to report any clinical progress or change to the Nurse in Charge. The post holder will carry out assigned tasks involving direct/indirect service user care and support service users under the supervision of a RN.
