Job summary
A community care provider in Stourport-on-Severn is seeking a Team Leader to manage care shifts and maintain quality standards. The role requires at least 2 years of experience in health and social care. The ideal candidate will lead the team, deal with emergencies, and ensure the well-being of residents. Flexibility in working hours is essential. This position offers an opportunity to make a significant impact in the lives of others.
Qualifications
- Minimum of 2 years working in the Health & Social Care sector.
- Ability to work a mix of early shifts, late shifts, and alternate weekends.
- Passionate about providing outstanding care.
Responsibilities
- Lead care shifts and handle emergencies.
- Create and update Care Plans and Risk Assessments.
- Complete staff rotas and quality monitoring checks.

Radis Community Care have an opportunity for a Team Leader to join our team based at School Gardens Extra Care Scheme in Stourport on Severn

As a Team Leader, you will be the first point of contact for residents and staff onsite as well as visiting health professionals. This can include:
- Leading the care shift and dealing with any emergencies that arise on shift
- Creating/updating Care Plans, Risk Assessments and other key documentation
- Auditing documentation such as MAR Charts, Daily Diary Records etc
- Completing staff rotas to ensure each shift is adequately staffed
- Completing quality monitoring checks such as spot checks, carers assessments & reviews
- Liaising with other Health Professions such as Doctors, District Nurses etc.
School Gardens is in the heart of the historic Georgian canal town of Stourport and is just a short stroll from Stourport's thriving town centre with its shops, restaurants and waterside leisure opportunities.
There are 60 beautiful one and two-bedroom apartments available to people aged 55 or over, set amidst a range of communal facilities including a cafe bistro and lounge, activity studio and a hair and beauty salon.
- Be able to work a mix of early shifts, late shifts, and alternate weekends
- Be willing to take part in an on call rota
- Worked in the Health & Social Care sector for a minimum of 2 years
- Be a caring individual who is passionate about putting the client first and ensuring we provide an Outstanding service we would want our own families to receive
- Have the Right To Work in the UK
Shifts - 7am-3pm, 2pm-10pm, and 9am-5pm. This role is mainly office based if no care shifts need to be covered.
