Band 5 Recovery Coordinator within Rough Sleepers and Alcohol in Trafford UK earning £20 per hour. This rewarding locum position is available for an initial 6 months with a view to extend. Full Time roles with flexible working or condensed hours are possible with manager agreement. As a Recovery Coordinator, you will be instrumental in transforming the lives of rough sleepers with alcohol dependency. What you will do:

• Conduct assertive outreach to engage with rough sleepers in Trafford, meeting them in their own environments.
• Develop and implement care plans to support individuals in overcoming alcohol dependency.
• Work collaboratively as part of a multi-disciplinary team, attending weekly MDTs and liaising with partner agencies.
• Recognise and report safeguarding concerns, ensuring that all actions adhere to policy guidelines.
• Manage a caseload autonomously while maintaining detailed and accurate records of all interactions and progress.

Requirements:

• Strong knowledge of safeguarding policy and the ability to recognise and address safeguarding concerns.
• Willingness to offer an assertive outreach approach, meeting patients where they are in the community.
• Proven experience working within a multi-disciplinary team environment.
• Ability to manage a caseload independently, organising and prioritising tasks efficiently.
