Senior Nurse Practitioner

We are looking to recruit a band 7 Senior Practitioner to work as part of the Urgent Treatment Centre team, who will be responsible for providing high‑quality health care and information to the public. An opportunity exists for an experienced and highly motivated practitioner with relevant adult or paediatric experience who is committed to making a valuable contribution to the further development of these pioneering services.

Main duties of the job
  • Act as a senior clinical professional within Widnes Urgent Treatment Centre, liaising with the UTC team and colleagues across the wider social and healthcare environment to promote, maintain and enhance effective multidisciplinary patient care.
  • Act as an independent practitioner who provides advice to patients and staff, contributing to triage, the Manchester triage system, history taking, assessment, diagnosis, plan of care and treatment of patients.
  • Possess and demonstrate extended skills in the management of a wide range of healthcare problems, i.e. minor illness and injury.
  • Maintain a high standard of clinical work when carrying out clinical procedures.
  • Be responsible for the safe custody of medication, including checking and administration.
  • Ensure compliance with our clinical governance framework, motivate colleagues, champion service‑user safety and dignity, and drive continuous improvement.
  • Exercise professional independent and clinical judgement in managing or referring clients to a variety of primary and secondary care services as appropriate.
  • Use leadership skills to drive the team to efficient and safe working practice and improved care for patients.
Managerial duties
  • Supervise and maintain high standards of professional care in accordance with current NMC Code of Professional Conduct, Scope of Professional Practice and ethical considerations.
  • Provide a supportive environment for staff, take responsibility for and perform clinical supervision and personal development planning.
  • Assist the Clinical Lead/Operational Manager in shaping and developing a nurse‑led service, which is welcoming and effective to the needs of service users.
  • Participate in the development of an effective nurse‑led service utilising a holistic, health‑needs approach.
  • Participate in the development, implementation and monitoring of policies, procedures and clinical guidelines as appropriate, proposing changes where necessary.
  • Supervise the supply of medicines by junior staff as appropriate via PGD.
  • Manage and co‑ordinate medicine stock control.
  • Day‑to‑day supervision of the team, including allocation, planning and organisation of the workforce, and work planning for the UTC, reallocating work and tasks as required to meet the demands of the service and patient needs, referring to alternative services as required.
Leadership

Fulfil the role of coordinator on a day‑to‑day basis or as and when required and assist the Clinical Lead in fulfilling the operational requirements of the service within budgetary limits. Delegate responsibility where appropriate and encourage a culture where flexibility and adaptability are positively encouraged. Support and maintain the focus of the nurse‑led service. Adopt and promote a positive approach to change. Actively participate in and promote the development of professional nursing practice.

Clinical duties

Exercise a high degree of clinical independence and professional judgement, treating independently within the Urgent Treatment Centre and, where necessary, referring to a range of primary organisations. Provide highly specialised advice when reviewing patients with undifferentiated and undiagnosed healthcare problems. Assess healthcare needs of patients, diagnose, treat and manage as appropriate. Referring to care services as appropriate. Possess proven skills in the management of a wide range of health‑care problems, i.e. minor illness and injury, wound care, chronic disease management. Provide health‑promotion education. Undertake a range of clinical procedures, which may include suturing, venepuncture, cannulation, ear syringing, slit lamp use, removal of foreign bodies. Participate in and undertake triage to ensure smooth progress of patients through the system. Initiate and conduct clinical investigations as appropriate. Supply and administer medications as appropriate via PGD. Exercise independent and delegated prescribing rights in accordance with current and legal and ICB criteria, and within the clinical situation. Identify at‑risk patients and manage appropriately in accordance with national and local policy and procedures, i.e. vulnerable children, adults and child protection. Collaborate with the Clinical Lead/Operational Manager in the co‑ordination of services and patients within the Urgent Treatment Centre. Act as an advocate for the patient at all times. Offer a complementary service to that of other health‑care providers within the local health economy. Utilising own professional knowledge and skills, act as a resource for other members of the Urgent Treatment Centre team. Empower colleagues and clients and demonstrate the ability to facilitate effective clinical practice based on best evidence. Possess a clear understanding of legal and ethical issues, professional accountability and responsibility. Using judgement and analytical skills when reviewing complex situations when reviewing patients, lab results and x‑rays with highly specialised needs and conditions, changing treatments as required. Able to adopt manual handling techniques safely when manoeuvring patients during the course of their care. Ability to concentrate frequently when assessing patients' needs and providing highly specialist care. Experience of working in highly unpleasant working conditions due to the nature of the role where patients can behave in a certain way or where body fluids such as open wounds are experienced frequently.

Registered prescriber responsibilities (if applicable)
  • To safely and effectively prescribe medication to patients, as an independent prescriber.
  • To apply knowledge of drug actions in prescribing practice.
  • To understand the legislation relevant to the practice of nurse prescribing.
  • To understand the roles and relationships of others involved in prescribing, supplying and administering of medicines.
Training and education
  • Participate and engage in the identification of education and training needs of the team, delivering on the job specialist training to students, new members of staff and to support medical students and postgraduate students.
  • Demonstrate a positive commitment to the training and education of pre and postgraduate students.
  • Identify and disseminate best practice through evidence, clinical guidelines and benchmarking.
  • Maintain and develop own clinical and professional expertise by participation in Clinical Supervision and Personal Development Planning, in accordance with Nursing and Midwifery Council Code of Conduct, and scope of professional practice.
  • Participate in formal and informal teaching sessions to promote clinical excellence within the UTC team.
Research and audit

Promote a research culture within the Urgent Treatment Centre, and actively promote the use, publication and dissemination of research evidence. Initiate and actively participate in audit and evaluation of the service eg, ED feedback, demographic service needs, laboratory results to promote change to practice. Initiating and participating in audit as and when required.

Communication

Engage in collaborative working with colleagues in primary, secondary and social care. Maintain and promote effective communication and working relationships with colleagues across the local health economy. Maintain effective systems of communication and referral both within the UTC and across primary and secondary care. Promote and maintain effective comprehensive clinical documentation in accordance with local and NMC guidelines for record keeping. Maintain confidentiality at all times in accordance with local and National requirements. Demonstrate professional skills in facilitating effective and clear communication in dealing with sensitive or volatile situations/issues i.e. child protection, alcohol/drug abuse, sexual orientation, racial and ethnic diversity, mental illness, domestic violence, physical deformity and others. Possess and demonstrate cultural competence by interacting effectively with clients from many backgrounds. Utilise the Trust e‑mail communication system. Utilise a variety of systems to communicate clinical information in accordance with the requirements of the data protection act (fax, telephone, letter and email). Utilise personal skills to facilitate effective and clear communication with clients to ensure collaborative health care concordance with recommended advice and treatment. Provide and receive highly complex communication in relation to highly specialised advice to patients that present at the UTC. This will include patients with learning disabilities and mental health issues, as well as patients where there are barriers to understanding.
