A leading health organization in Wales is seeking an enthusiastic Band 5 Registered Nurse to join their Home First Team in Wrexham. The position involves working within a supportive team, providing nursing care, and facilitating safe patient discharge. Candidates must be NMC registered and have experience in holistic nursing care. The role offers full-time hours, with part-time options considered, and requires effective communication skills. Knowledge of Welsh is desirable.

An exciting opportunity has arisen for an enthusiastic and motivated Band 5 Registered Nurse to join the multi-disciplinary Home First Team based in Wrexham.
The post is 37.5 hours covering 5 days a week between 10am and 6pm, to meet the service requirements. Part time / job share would be considered.
The successful candidate will be based within the Emergency Quadrant at the Wrexham Maelor Hospital. They will also be responsible for the ongoing screening of patients to facilitate safe and timely discharge from the Emergency or Assessment Units through the Discharge to Assess model of care.
The successful post holder will work within a supportive multi-disciplinary team to promote the Home First ethos through ongoing therapeutic interventions in the patient's own environment.
The successful candidate will act as a link between the acute and community services and will work alongside our established Community Resource Team's in both Wrexham and Flintshire.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
