A leading healthcare provider is seeking a Long Term Conditions Nurse to join its Primary Care team at HMP Full Sutton, located in York. This full-time role offers a salary up to £49,175 FTE and entails responsibilities such as coordinating care for patients in custody and ensuring the delivery of high-quality health services. Applicants should be registered nurses with experience in long-term conditions and knowledge of health and social care models.
We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Full Sutton, a category A men’s prison located in Full Sutton, York.
This rural site offers its own parking, an on‑site gym, café, team building days and well‑being events each year, regular 1‑1’s and a strong scope for development including vaccination training and opportunities to gain advanced qualifications.
Although not essential, experience in a hospital, community or prison environment is advantageous.
