We are seeking a highly skilled Advanced Nurse Practitioner to join our Clinical Assessment Service (CAS), supporting patients across Surrey. This pivotal role involves assessing patients remotely via telephone, planning and administering treatment, and making timely decisions regarding discharge or referral, all within your clinical scope of practice. You will be a key part of a multi-disciplinary, fast-paced environment, operating in line with GP surgeries and Practice Plus Group protocols.
In this role, you will manage your own virtual caseload, providing high-quality, evidence-based patient care, including prescribing where appropriate. You will work closely with other healthcare professionals to ensure patients receive the most appropriate outcomes whether that's self‑care advice, referral to face‑to‑face services.
