Staff Nurse

We are currently recruiting a Band 5 Nurse, 23 hours a week, to be part of our team at Evesham Community Hospital, which is a 50 bedded stroke and general rehabilitation unit. The nursing staff work alongside the therapy team and adopt a multidisciplinary team approach to providing rehab to the patients.

We require an innovative, enthusiastic, motivated individual interested in all areas of rehab. You will be required to plan, implement, evaluate and take responsibility for the nursing care of the ward patients on a daily basis. You will be expected to facilitate and support the patient and their families throughout their pathway from admission to discharge.

Main duties of the job

As a Staff Nurse you will be responsible for providing care to people admitted to our wards for rehabilitation. We work closely as a multi‑disciplinary team. Our focus is promoting the comfort and wellbeing of patients, enabling them to live as independently as possible. Evesham is a friendly team to work with. As a team we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice. Our nurses are required to facilitate the day to day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families and carers to provide 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

Job responsibilities

The post holder will:

• Be responsible for the assessment, planning, implementation and evaluation of patients care within the Ward/unit, working in accordance with the philosophy of patient centred care and to NSF and other Trust standards.

Will have competencies in all of the following and will:

• Plan and deliver individual nursing care under direction of senior staff.
• Work within the NMC Code of Professional Conduct.
• Assess, plan, implement and evaluate patient focused nursing care.
• Document care given and maintain contemporaneous records.
• Maintain records in accordance with the Trust's policy and that of NMC Code of Professional Conduct.
• Assist in implementation of health promotion strategies, as agreed by Ward Manager.
• In consultation with senior qualified staff, provide and monitor patient centred care within Trust agreed standards.
• In collaboration with other team members organise the reception and discharge of patients ensuring effective communication in transfer of information.
• Carry out delegated nursing care.
• Use and participate in research / audit in care delivery.
• Adhere to and promote all clinical practice, health and safety and corporate Trust Policies and Guidelines.

Person Specification

Knowledge

• Confidence to teach student nurses, patients, carers and members of the multidisciplinary team.
• Understanding of evidence-based nursing, multidisciplinary working, clinical governance.
• Awareness of audit.
• Evidence of on-going continuing professional development.
• Experience of audit.
• Knowledge and understanding of Mental Capacity and DOLs.

Experience

• Experience gained during pre‑registration training.
• Experience gained working in the frailty care setting.

Additional Criteria

• Able to carry out physical requirements of the role e.g. using mechanical hoists to move patients.

Qualifications

• RGN/RMN.
• NMC Registered.
• I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
• Accredited courses in Palliative Care, Tissue Viability, Diabetes, Dementia.
