Job description

The Home First service brings together nurses,paramedics,therapists,healthand therapy assistants, to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient's own home.

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission. Home First will, whererequired, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

You could become part of our exciting service, bringing the unique benefits of a wraparound recovery service from hospital discharge. The principle of the service ensuring that recovery involves working towards the person being as independent as possible through enablement.

Main duties of the job

Holistically assess, plan, monitor, evaluate and discharge patients whilst working in an integrated manner with the multi-disciplinary team.

Empower patients to work as partners in their care, respecting individual choice and difference and encouraging self-care where able. Support patients in a programme of self-care/enablement.

Facilitate patients and their families to plan for the future and improve end of life care ensuring that choices are reflected in personalised care plans.

Identify significant change in patient's physical, psychological and social status, acting on findings, seeking guidance and/or reporting to the appropriate member of the multi-disciplinary team.

Demonstrate effective time management by prioritisation and delegation of patient care, promoting continuity whenever possible and working in collaboration within the team.

Demonstrate commitment to continual professional development, participating in clinical supervision and working within the NMC code and Sussex Community NHS Foundation Trust policies demonstrating awareness of personal accountability for own actions.

Demonstrate a high standard of clinical information collation and documentation, adhering to Trust and national guidelines pertaining to the collection and storage of clinical data.

To attend all mandatory and statutory training as appropriate to role.

Job responsibilities

Communication and working relationships

Working in partnership, the post-holder communicates effectively with patients and their carers, the multi-disciplinary team and other key stakeholders on a range of complex health care matters, issues, ideas and situations.

Assist in maintaining a safe working environment for self, colleagues, patients and others, report any accidents or untoward occurrences as per policy and undertake any investigations as required for untoward incidence, complaints, risk management and health and safety issues.

They act as an information resource for patients, multi-disciplinary colleagues and other stakeholders. Responsible for signposting and referring individuals, families, carers and communities to the most appropriate agencies/services to support their needs.

Contribute to ensuring effective liaison and communication with other professional groups and outside agencies, acting as an information source, and work in partnership in the interests of patients.

Person Specification
Qualifications/professional registration
  • Experience of dealing with a wide range of people at all levels, including clients, carers, therapists, health and social care colleagues and other professionals
  • Mentorship Qualification or work towards
  • Demonstrates commitment to and recent evidence of further education and professional development
  • Experienced in a variety of clinical settings
  • Experience of working within the Community
  • Experience of EOL patients and EOL care
Skills and Knowledge
  • Knowledge of Clinical Governance, clinical audit and clinical supervision, Professional codes of Conduct
  • Demonstrates competency in a range of clinical skills e.g. wound management, catheterisation skills, medicines management and administration, palliative and end of life care including management of syringe drivers
  • Knowledge of current local policies informing community health and social care services
  • Is able to demonstrate the application of research based practice
  • Promote independence and choice in undertaking patient care
  • Ability to work as an effective member of the multidisciplinary team
  • Good communication and negotiation skills
