COMPENSATION: $39.23/hour

**This is a Relief Position – 8 - 10 hours/week**

LOFT Community Services is a unique and dynamic charitable organization that supports people living with complicated issues such as mental and physical health challenges, substance use challenges, poverty and homelessness. Serving approximately 19,633 annually and providing 1,861 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. LOFT is also recognized as a Nonprofit Employer of Choice Award (NEOC) recipient.
To help fulfill its mission, LOFT is hiring a Relief Registered Nurse.

Job Summary
The RN will be part of an interdisciplinary team, who is expected to ensure the safety and security of
the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RN will hold a unique and highly independent role as the clinical lead in an innovative practice setting.The RN will lead a team of Registered Practical Nurses to ensure that applicable delegated or controlled acts are safely and consistently delivered by Personal Support Workers (PSWs). The RN will be responsible for working with individuals from diverse backgrounds, lead clinical supervision of clients in the program; and providing training and leadership around the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The RN is directly responsible for clinical assessment and monitoring of client needs, assigning and monitoring controlled act delegation activities between RPNs and PSWs, and providing training and supervision to PSWs in the performance of delegated and controlled acts.

Back to Home Penetanguishene LOFT provides a compassionate and comprehensive assisted living solution for older adults grappling with complex mental and physical health challenges, including substance use, homelessness, dementia, and other neurological disorders. Our services cater to seniors aged 55 and above who require permanent subsidized housing with specialized assistance. Eligibility includes those designated as Alternative Level of Care (ALC) or at risk of being designated, and who need around-the-clock support to remain safely at home. Referrals are managed through Centralized Intake, with sources typically being hospitals in the North Simcoe Muskoka (NSM) region. Once referred, potential residents undergo an intake process by LOFT, which includes a facility tour and acceptance of the housing offer. Our facility possesses 27 beds, comprising 24 permanent and 3 transitional options, offering both private and shared accommodations. Additionally, we provide Hub and Spoke services, which are community-based support designed for those who, while able to live at home, need ongoing care with personal supports available 24/7. This system is supported through our existing Assisted Living buildings, ensuring that clients receive crucial support while remaining in their own homes. For those under 55 requiring significant assistance with activities of daily living (ADLs), we consider cases on an individual basis, ensuring that every resident’s unique needs are met with tailored care.

What You Will Do
  • Conduct clinical assessments and develop nursing care plans
  • Monitor RPNs’ nursing practice in collaboration with LOFT Program Directors and provide, in specific cases, direct nursing care with medications as defined by the College of Nursing Practice including:
    • administration of medications through oral, subcutaneous, topical, natural body orifices, intracutaneous, tubing routes
    • transcribing medical orders as given by physicians in writing or by telephone
    • administration of routine medications including narcotics
    • administration of routine p.r.n. medications, effect of such and identify when the order needs to be adjusted
    • maintains security of medication system and storage
    • orders medications as required and maintains MARsystem
    • follows all policy and procedures as outlined by the Pharmacy
  • Assign and monitor delegation activities between RPNs and PSWs.
  • Provide training and supervision to PSWs in the performance of delegated and controlled acts for clients where direct RN involvement is most appropriate.
  • Provide advice, coaching and support to staff regarding medical/healthcare questions and concerns where appropriate.
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community Services.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners
  • Attend and actively participate in all required training.
  • Serve as back up to Registered Practical Nurses, asrequired.
  • Follows standard infection control practices and enhancedprecautions as instructed.
  • Effectively participate as a member of the LOFT Team;attend staff/team meetings, client care conferences,other meetings etc. as required.
  • Complete other duties, from time to time, as assigned bythe Director.
What You Bring:
Must Haves
  • Registration as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario, RNAO member.
  • University undergraduate degree in Nursing
  • Minimum of three years’ experience in home and community-based nursing
  • Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges who have been longer term hospital patients
  • An understanding of recovery and psychosocial rehabilitation principles and the therapeutic
    relationship
  • Experience delegating controlled nursing acts (e.g. Gtubes, wound care, diabetes management, mechanical lifts, etc.) to unregulated health care providers
  • Demonstrated ability to use knowledge transfer techniques to support practice delegation and good understanding of learning needs of unregulated care providers and interdisciplinary point of care providers
  • Strong clinical practices with ability to champion evidence-based best practices
  • Demonstrated ability to communicate effectively and problem-solve complex clinical situations
  • Clear general understanding of the continuum of care in the community sector, the Ontario health system and knowledge of social services and community resources and supports for adults and seniors
  • Able to recognize safety issues, follow safe work practices, advocate for client care and safety concerns
  • First Aid and CPR certification
  • Self-directed, results oriented and capable of working independently as well as in a team environment.
  • Ability to collaborate with other members of circle of care to solve problems and meet client needs
  • Demonstrated commitment to modelling lifelong learning and professional growth
  • Good oral and written skills.
  • Excellent computer skills.
  • Ability to work effectively in a team environment.
  • Ability to communicate in a different language, an asset.
  • Understanding of cultural competency and anti-oppression framework.
  • Reflective of a multi-barriered group, as serviced by the program, a strong asset.

Work Environment
  • Physically able to perform demanding body mechanics (i.e. lifting, transferring, Hoyer, etc.)
  • Occasional evening and weekend hours may be required.
  • Physically able to perform occasional client transfers in the home (as part of the assessment process).
  • Extensive travel is involved and use of own vehicle is necessary
