Due to a recent retirement of the post, NHS South Yorkshire Integrated Care Board (SY ICB) Sheffield Place has an exciting opportunity to join the Continuing Healthcare team. We require a registered nurse (RLDN) to support our dynamic and welcoming Continuing Healthcare team.

The Sheffield Continuing Healthcare team are an innovative and friendly team who deliver a service to the population of Sheffield. This is a great opportunity to be involved in a team working with the Local Authority and other partner organisations, towards a more integrated approach in how we assess and meet an individual's ongoing care needs.

The post holder will be responsible for:

1. Coordinating the Multi-disciplinary team (MDT) eligibility assessment of an individual's needs regarding eligibility for CHC.
2. Identifying and securing the involvement of the MDT, which will assess the individual's needs and complete the Decision Support Tool (DST), supporting MDT members to understand their roles in the multidisciplinary assessment process.
3. Facilitating Continuing Healthcare assessments for patients across all aspects of care. This requires high assessment skills, tact, diplomacy, and effective liaison with colleagues, healthcare professionals, patients' carers, and family members. A Registered Nurse qualification is required.
4. Supporting the person and their representatives to participate fully in the eligibility process, ensuring they understand the process, have access to advocacy or support if needed, and organizing the process to maximize their participation.

• Providing highly specialist advice on the National Framework for Continuing Healthcare eligibility in complex situations, including care procurement, personal health budgets, and ongoing reviews.
• Ensuring a clear timetable for decision-making and completion of DST processes according to the National Framework.
• Acting as an impartial resource for the MDT and individuals on policy or procedural questions.
• Ensuring timely submission of MDT recommendations for eligibility for ratification.
• Ensuring timely 'End of life' care procurement and management for those on the Fast Track pathway.
• Completing reviews in line with the national framework and coordinating ongoing care package reviews, liaising with the MDT to ensure safety and compliance with Sheffield's Care Principles.
• Communicating complex information and handling sensitive conversations to support understanding.
• Providing healthcare advice to secondary and primary care, nursing homes, and other settings, developing effective communication networks.
• Interpreting national and regional guidance related to the National Framework and CHC developments.
• Applying professional judgment when communicating on behalf of the CHC service.
• Collaborating with health professionals, colleagues, and support staff to ensure quality delivery.
• Delivering training to community and hospital staff on CHC assessment and management, based on current research and guidance.
• Operating as part of a team, providing cover and support to colleagues to meet patient needs consistently and according to best practices.
• Managing an active caseload, providing care management for eligible individuals.
• Identifying appropriate care placements and preparing packages of care, presenting these to the Sheffield resource panel.
• Representing care-managed individuals at MDT meetings, leading assessments such as mental capacity and best interest decisions.
• Engaging in risk management and safeguarding, liaising with other professionals and attending meetings as required.
• Working independently, traveling as needed within and out of Sheffield.
• Supporting and mentoring students and less experienced team members.
• Adhering to confidentiality, GDPR, and NMC standards in recordkeeping and practice.
• Promoting equality and diversity, complying with related policies, and actively supporting these principles.
• Upholding transparency, openness, and candor in all practices.
• Maintaining safeguarding knowledge and compliance with relevant policies.
• Reporting infection prevention issues appropriately.
