The purpose of this role is to work as a qualified Health Care Assistant, providing quality clinical and administrative support efficiently to patients across Tower Hamlets, Newham, Waltham Forest and Havering.

Travel by car or motorcycle to various borough locations is required. The successful candidate must be able to travel within the specified areas and conduct home visits.

• Provide a professional service to our multilingual, diverse patient population.
• Work with GP/PCN Care Coordinators and GP Assistants to identify patients lost to follow‑up and not served by existing diabetes or SMI services.
• Perform phlebotomy and point‑of‑care testing (e.g., anticoagulation clinics).
• Write back/discharge to GP or update EMIS directly as appropriate.
• Liaise with participating practices to book patients for reviews.
• Review patient goals and complete appropriate onward referrals (foot health, education, weight‑management programmes, etc.).
• Support patients through referrals and signposting to services such as dietitians, gyms, walking groups.
• Consider patients’ mental health and wellbeing; refer to social prescribers, wellbeing coaches or talking therapies when indicated.
• Co‑ordinate recent discharges from hospital and liaise with DSN.
• Engage in shared decision‑making with patients, involving personalised care roles where suitable.
• Perform chaperoning duties and other clinical activities as per written guidelines.
• Carry out decontamination and sterilisation procedures.

• Maintain infection control standards and update medical records.
• Report concerns to the practice team that cannot be resolved during the HCA consultation.

• Maintain accurate written and computerised records in line with NHS NEL policies.
• Collect and prepare statistical information using existing recording systems.
• Attend meetings and communicate effectively outside of meetings.

• Participate in annual performance reviews and maintain a record of personal and professional development.
• Take responsibility for own learning and performance improvement.
• Attend in‑service training and other required training sessions.
• Identify and pursue training needs to enhance quality care.
• Assist with the training and development of other personnel as required by the line manager.

Essential:

• Qualified Health Care Assistant.
• NVQ Level 3 or higher in health and social care.
• At least 2 years’ experience in a GP or hospital setting.
• Care Certificate with all 15 standards signed off.
• Ability to travel within boroughs and conduct home visits.

Desirable:

• Experience with INR or point‑of‑care testing.

• Maintain confidentiality at all times.
• Act as a chaperone with appropriate training, documenting each occasion using the practice template.
• Maintain health and safety standards at all times.

• Roaming role requiring travel across Waltham Forest, Tower Hamlets, Havering and Newham.

Subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975; a D&B check will be required.
