Community Mental Health Practitioner - Crisis Team

Are you a Mental Health Practitioner (RMN/OT or Social Worker) looking for a new opportunity to work in an organisation that will help you develop and achieve your goals to become the best you can be? If so, you're in luck.

Midlands Partnership University NHS Foundation Trust has an opportunity to join our Crisis Resolution/ Home Crisis Treatment team in Southwest Staffordshire based at Haywood Lodge, Stafford as a Community Mental Health Practitioner.

We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week.

This is a 24-hour service and team members are all required to work rostered shifts including weekends and nights and you must have access to suitable transport, to cover a large geographical area for your shifts.

Main duties of the job
  • Assessing mental health crises in individuals referred to the service and supporting the delivery of safe and effective home treatment for an identified client group and in accordance with a wellbeing and recovery action plan
  • You will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved.
  • You will assist in the delivery of a high standard of nursing care to service users referred to the team.
  • You will implement agreed systems for the monitoring of effectiveness of care.
  • You will participate in the education and management of junior staff.

Person Specification
Qualifications
  • Be a Registered mental health Nurse or Allied Health Professional on NMC or HCPC Register
  • Post‑registration experience, evidence of formal courses and training at diploma/degree level.
Experience
  • Evidence of extensive post‑registration experience with working with adults experiencing mental health related difficulties
  • Experience of integrating care delivery across multi‑disciplinary/multi‑agency teams
  • Experience and ability to carry out clinical audit
  • Experince working in CRHT
