Job summary

An exciting opportunity has arisen for an enthusiastic, passionate and motivated perinatal community mental health nurse to join our expanding Perinatal Mental Health Team on a fixed term part time contract that will be covering maternity leave.

We are a specialist service that strives to deliver excellent standards of care to expectant and new mothers with severe and complex mental health problems. The team is multi-disciplinary and works directly with women and their families providing assessment, interventions and support for pregnant women and women within the postnatal period with a range of mental health needs.

You will join a friendly multidisciplinary team who are innovative and have a patient centred approach at the heart of their delivery. There are extensive training opportunities, and regular reflective practice and supervision.

The PMHT covers Telford and Shropshire area. The team base is in Bridgnorth, and we use other MPFT facilities across the county along with blended working options.

Perinatal and maternal mental health services in Shropshire won The Positive Practice in Mental Health (PPiMH) Award in 2021 and were commended in 2025.

Main duties of the job
  • Provide high quality comprehensive specialist assessment.
  • Management of a complex caseload.
  • Develop individual care plans in conjunction with women, families and wider MDT.
  • Undertake risk assessment and management.
  • Utilise outcome measures as part of the care planning and evaluation process.
  • Contribute to service development.

Job responsibilities
  • Take responsibility for the management of a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including pharmacological and psychologically informed interventions.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
  • Assist in ensuring that the aims and objectives of the ward/department (as set by the responsible nurse) are fulfilled and to identify factors which may inhibit these from being achieved.
  • The post holder will be required to work within the NMC Code of Practice and professional guidelines.
  • Maintain an up‑to‑date level of professional competence within the specific environment.
  • Assist in the implementation of evidence‑based nursing practice and participate in research and other projects as required.

    • Registered mental health Nurse

    • Evidence of extensive post‑registration experience with working with adults experiencing mental health related difficulties.
    • Experience of working with women experiencing perinatal mental health needs/knowledge of demographic risk factors for this client group.
    • Experience of working with people who have difficulty in engaging with services and have complex mental health needs.
    • Experience of working in community setting and making decisions autonomously.
    • Experience of assessing, planning, co‑ordinating and managing people with mental health problems in a community setting.
    • Experience of integrating care delivery across multi‑disciplinary/multi‑agency teams.
    • Experience of leading/contributing to CPA/SAP process.
