Job summary

An exciting opportunity has arisen for an experienced, committedAdvanced Nurse Practitionerto join our PCN in Billingham and Norton.

ThisAdvanced Nurse Practitionerrole will involve working alongside a multidisciplinary team, including GPs, Clinical Pharmacists and Personalised Care staff to provide care and treatment to patients.

The successfulAdvanced Nurse Practitionerwill work as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients. This may be by face-to-face or remote consultations.

Main duties of the job

What we are looking for;

-Registered Adult Nurse registered with the Nursing & Midwifery Council (NMC) or Paramedic registered with the Health Care Professions Council (HCPC)

- Hold an Advanced Practitioner qualification

- Can show evidence of using autonomous physical examination and assessment skills in practice

- Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children

- Knowledge and experience of QOF desirable

About us

Our mission in Billingham & Norton is to strengthen, develop and support local general practice, to enable the delivery of high-quality care to local people.

Our PCN is a network of seven practices with a combined list size of over 51,000 patients. We work collaboratively across all of our practices and have a strong working relationship with our GP Federation.

The team you will be working with as an Advanced Nurse Practitioner;

  • A fully staffed multi-disciplinary team within the PCN
  • Across the 7 practices in the vicinity, you will be supported by GPs, ANPs, Practice Nurses & HCAs
  • The team are a progressive and collaborative group, ready to welcome on board any successful additions

The successful post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

In accordance with the patient lists, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

Key Responsibilities:

To deliver a high standard of patient care as an Advanced Nurse Practitioner (ANP) in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

Working in a flexible approach and in collaboration with the GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.

Working within their own professional boundaries, responding effectively to patient need and ensuring patient choice and ease of access to services.

To provide education and training to other colleagues.

To proactively support future development of the PCN and share the PCN values of excellence, responsibility, integrity, development and commitment.

Provide a first point of contact within the PCN for patients with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan, either as face-to-face appointments or via the telephone.

Be accountable for own actions and work within own scope of practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.

As an independent prescriber, ensure delivery of safe, effective, and appropriate medication in a cost-effective manner, following best practice/national guidance and as defined by current legislative framework.

Provide safe, evidence-based, individualised, and holistic care to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.

Promote health and well-being through health promotion, health education, screening and therapeutic communication skills.

Refer patients directly to other services/agencies as appropriate.

Work with multi-disciplinary team within the PCN and its practices to promote integrated and seamless pathways of care.

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery e.g. QAIF and enhanced services.

Opportunity to develop an area of special interest within the practice to support improved outcomes for practice population.

Audit outcomes against standards and initiates learning/change where necessary.

To actively implement safeguarding protocols for vulnerable adults and to know who to refer to and how to escalate any concerns promptly.

To carry out all duties in a professional manner and adhere to all PCN and practice protocols, policies and procedural guidelines always.

Maintain their professional registration through continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.

To work with GPs in identifying issues related to clinical care which need to be further discussed within the practice.
