Major Responsibilities
Deliver public health nursing services in the French language to Toronto families, schools, and communities
Provide public health nursing services for individuals, families, groups, schools, and the community according to program-specific policies and procedures
Conduct relevant screening and health/service assessment at the individual/family/school/community level and develop appropriate plans
Assess and make referrals to other services, health professionals/agencies, and direct clients to community resources
Deliver interventions from a health promotion and behaviour change perspective, based on the health needs assessment
Maintain service delivery records and other necessary documentation according to TPH policies and procedures
Collaborate and consult with community groups, coalitions, and agencies to assess health needs and plan/deliver TPH programs and services
Facilitate community partnerships and liaise with other professionals to identify health needs and develop action plans
Represent TPH on assigned community networks and committees
Mentor and provide training, guidance, and support internally to staff and externally to school staff/parents, students, and peers
Prepare and present activity reports, data, and other reports, and contribute to reports needed for external funding sources
Key Qualifications
A Baccalaureate degree in nursing from a recognized university and a valid license from the College of Nurses of Ontario
Experience in public health nursing or community nursing, and in providing health promotion services
Experience working with priority populations and ability to create safe and inclusive environments
Written and verbal fluency in French at a professional level
Possession of a valid Class 'G' Ontario Driver's License and access to a vehicle
Knowledge of population health approaches, client engagement principles, and health promotion theory
Excellent bilingual oral and written communication skills and ability to review and translate program materials
Excellent assessment, consultation, counselling skills, and group facilitation skills
Ability to work using anti-oppressive practices and harm reduction approaches
