The Patient Care Coordinator serves as a vital link between patients, healthcare providers, and healthcare facilities, ensuring that patients receive the best possible care. This role involves managing patient appointments, handling logistics, and ensuring that all necessary information is properly communicated among healthcare professionals and patients. Responsibilities

Requirements

• Bachelor's degree in healthcare administration, nursing, or related field preferred.
• Previous experience in a healthcare setting or in patient care coordination roles.
• Excellent communication skills, both verbal and written, for effective coordination.
• Strong organizational skills to manage multiple tasks and priorities simultaneously.
• Ability to maintain confidentiality of patient information in compliance with regulations.
• Proficiency in using healthcare management software and electronic medical records systems.
• Compassionate attitude and a patient-focused approach to care coordination roles.
• Ability to work independently as well as collaboratively within a diverse healthcare team.
