Qualifications

  • Current Registered Nurse registration with Australian Health Practitioner Regulation Agency.
  • Experience in staff training and competency assessment.
  • Knowledge of current National Aged Care and NDIS Standards.

Responsibilities

  • Conduct comprehensive clinical assessments at Clients' residence.
  • Develop individualized care plans in partnership with the Care Partner.
  • Monitor Client wellbeing and service outcomes.
  • Deliver training to care staff and assess competency.
  • Provide phone consulting to care staff regarding clinical issues.

We are seeking aRegisteredNurse to join our caring and dynamic team to provide clinical guidance for safe and effective in‑home support services to our valued Clients

Duties and responsibilities include :
  • Conduct comprehensive clinical assessments at Clients residence; incorporate input from Clients familyand carers reports from medical and Allied Health professionals and feedback from care staff
  • In partnership with Care Partner / Client Services Manager (non‑clinical) develop suitable individualised care plans to address the needs of each Client
  • Monitor Client wellbeing and service outcomes to determine any changing clinical needs via service feedback from care staff healthcare summaries and hospital discharge summaries
  • Deliver orientation annual refresher and Client‑specific training to care staff; assess and certify staff competency
  • Provide phone consulting to care staff regarding clinical or medical issues arising during Client care; manage incident reporting of any medical incident including monthly review / analysis of underlying causes and care improvement opportunities
  • Act as Subject Matter Expert for all clinical issues standards compliance; contribute to the development of best‑practice care plan templates; manage related external relationships including clinical liaison and reporting for Just Better Care Australia Aged Care Commission and NDIS

What you will need :
  • Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
  • Evidence of continued professional development in accordance with the AHPRA standards
  • Demonstrate a professional confident approach to understanding Clients needs and ensuring positive outcomes
  • Awareness of current National Aged Care and NDIS Standards
  • Experience in staff training and competency assessment
  • Current Australian Drivers Licence and reliable registered and insured vehicle
Desirable :
  • Previous employment experience in community care or disability / aged care or community services sector
  • Knowledge of and experience in developing health care plans to the clinical standards required by Aged Care Quality standards and NDIS requirements
  • Certificate IV in Training and Assessment
  • Knowledge of continuous quality improvement principles
