We are expanding our Rapid Response Community Nursing Team in Luton to support community urgent crisis response requirements and the development of Virtual Wards. This presents an exciting opportunity for experienced and skilled Registered Nurses to join the team. The Rapid Response Team in Luton provides high quality nursing care to maximise health status, minimise ill health and disability, and support independent living. The team works with the acute hospital, ambulance service, GP practices and other providers to enable care at home and avoid unnecessary hospital admissions. The team also supports District Nursing with unscheduled/urgent care in and out of hours across the Luton area. Shifts cover 08:00 to 21:00.

Responsibilities
  • As a senior member of the team, lead the team by undertaking clinical assessments, planning and delivering care, demonstrate effective leadership skills and contribute to continuing service development to meet business and operational needs.
  • Provide clinical support and leadership to Community Rapid Response services; contribute to the continued development of the service in line with local priorities and national strategies.
  • Provide acute clinical assessment and treatment to patients in their own home, including residential homes, undertaking the planning, assessment and evaluation of patient care packages and arranging onward referrals as required.
  • Ensure that through partnership working the service liaises with community, primary and secondary healthcare professionals to provide a holistic care package designed to meet individual needs and to prevent unnecessary hospital admission and facilitate timely discharge.
  • Work with the Team Lead to provide leadership and line management support to team members to deliver safe, effective care and maintain pathways and processes that support safe care.
  • Ensure safe delivery of care via local policy and procedures, including telephone assessment and prioritisation of patient referrals.
  • Develop specialised care packages for patients, plan and deliver prescribed therapy in patients’ homes, including intravenous therapy as prescribed.
  • Liaise with Discharge Planning and medical staff to manage safe discharge of patients; liaise with GPs and the multidisciplinary team to plan care packages that support patients at home and avoid inappropriate admissions.
  • Ensure appropriate staff cover for management of caseload/service and take action to provide a safe, high-quality service.
  • Provide Clinical Nurse Triage to support single point of access; recognise and assess complications and refer to physicians/GPs/Specialists as required.
  • Prioritise workload, review caseload and allocate tasks/visits; provide education to patients on treatment, side effects and outcomes to empower self-management.
  • Prescribe medications where applicable in line with independent nurse prescribing guidelines following accredited training and competence.
  • Ensure patients are treated with care and compassion, provide support to those coping with acute and/or long-term conditions.
  • Identify and manage unpredicted crises (staff, caseload, support network) and escalate to senior managers as required.
  • Collaborate with partners to respond and plan care packages at home to avoid hospital admissions and support timely discharge.
  • Support leadership and development of the service and continued improvement of patient care pathways.
Qualifications and Experience
  • Registered Nurse with current UK professional registration; experience in autonomous working, clinical assessment and decision making; strong clinical leadership skills.
  • Experience in providing clinical leadership and developing services; ability to work in a dynamic environment and deliver safe, high-quality care.
  • Personal development planning and commitment to staff development.
  • Ability to work with multi-disciplinary teams and across boundaries to achieve shared goals.

  • On 1 April 2025, Cambridgeshire Community Services NHS Trust and Norfolk Community Health and Care NHS Trust will form a group with a single board while remaining two separate employers until further notice; the specific role being advertised will be with Cambridgeshire Community Services NHS Trust.
