A leading community healthcare provider in Westminster seeks a Band 7 Occupational Therapist or Physiotherapist to lead community therapy pathways. You will work with a multidisciplinary team to ensure patients receive holistic care post-discharge from hospital. Responsibilities include liaising with hospital teams, conducting assessments, and managing patient rehabilitation. The ICT covers several community pathways involved in supporting hospital flow and supporting patients in the community in Wandsworth. These are:
The B7 will lead one of the primary pathways above, with the expectation to cross-cover across all pathways within the intermediate care team. Staff are expected to utilise CLCH bases across Wandsworth if necessary. The ICT provides a 7 day a week service. Examples of duties are below:
Risk management, disability management and rehabilitation are key components of the service delivery. The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills. The team consists of Physiotherapists, Occupational Therapists, rehabilitation assistants / healthcare assistants & phlebotomists in a multidisciplinary team.
