Job summary

Qualifications

Overview

In this varied and rewarding position, you will have the opportunity to support both nursing areas and therapy services, working closely with our multidisciplinary team to deliver high-quality, patient-centred care. Comprehensive and appropriate training will be provided to ensure you feel confident and supported in your role. HCRG is advancing a major transformation to embed the Integrated Neighbourhood Health vision set out in the NHS Long Term Plan. By fostering stronger collaboration across services and adopting innovative ways of working, we aim to create healthier, more resilient communities and ensure that health and care resources are used where they have the greatest impact. Interviews will be held via an assessment day on 27th February, from 10:00am. Successful applicants will be invited to attend and take part in a range of activities designed to help us get to know you and your skills.

Responsibilities
  • Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
  • To keep the patient at the centre of their care, ensuring patient and carer participation in decision making.
  • Responsibility for, and supporting, the day-to-day care of patients, including monitoring.
  • To communicate with individuals about routine and daily activities, this may include overcoming difficulties (e.g. hearing/language).
  • To work within a multi-professional team and to feed back service user progress, liaising with colleagues, families, carers and other agencies.
  • To assist registered and unregistered staff, referring to registered staff when necessary.
