A dedicated charity organization in Doncaster seeks a part-time Discharge, Support and Admission Prevention Worker to assist older and vulnerable individuals during their transition from hospital back to the community. The role requires strong communication skills, experience in community work, and the ability to provide empathetic support. This position offers the chance to make a significant impact on the lives of individuals needing support post-discharge.
We are delighted to be supporting SCCCC, a charity very close to our heart, on their search for a Discharge, Support and Admission Prevention Worker to join their growing team in Doncaster. This position sits within the Home from Hospital team, and supports individuals when they have been discharged from hospital. This role will make an enormous difference to the lives of people in Doncaster. This people-focused role will directly support older and vulnerable people in the Doncaster community. Some service users may be lonely, and socially isolated and need a helping hand when they are being discharged from hospital. This role is part time working either 21, or 28 hours per week and is well suited to a friendly and caring individual.
