Job summary
A healthcare staffing agency is looking for an experienced Adult’s Social Worker for a contract role in the Fylde area of Lancashire, focusing on Hospital Discharge support. The position offers hybrid working arrangements, and applicants will work closely with multidisciplinary teams. Candidates should have knowledge of CHC processes and be proficient in conducting care home reviews. A dedicated consultant and comprehensive support are provided throughout your assignment.
