Libertatem Healthcare Group is seeking a support worker with experience working with brain injury. The client is in his 20s, lives in his property, and has sustained a frontal lobe brain injury due to an RTA. He is mobile and likes to have an active lifestyle. The role requires a driving licence as the client owns a vehicle and expects staff to drive.

Hours

1:1 – 24/7 (Bank holidays observed)

Main Tasks and Duties
  • Supporting independence and continued rehabilitation.
  • Provide personalised support in a sensitive and confidential manner according to the individual’s support plan.
  • Understand and follow care plans, seeking clarification from the health‑care coordinator or clinical lead nurse as necessary.
  • Participate in monitoring and review of Service Users’ care plans and work in a professional manner with the multidisciplinary team.
  • Promote self‑care and self‑management to achieve maximum independence.
  • Assistance with domestic tasks such as laundry, ironing, and household cleaning.
  • Prepare food and drinks, kitchen cleaning and hygiene, assistance with shopping and menu planning.
  • Assist Service Users to attend appointments, shop, collect pensions, etc.
  • Provide emotional support to increase confidence and motivation.
  • Assist with moving and handling equipment to promote independence and safety.
  • Monitor skin integrity and raise any changes with the clinical lead.
  • Administer medication as designated in the care plan in accordance with company policies, including monitoring self‑medication or prompting.
  • Assist with rehabilitation activities directed by the multidisciplinary team.
  • Monitor general health and well‑being, responding to indications of autonomic dysreflexia or infection.
  • Deliver support sensitively respecting culture, disability, race, age, sexuality, marital status, transgender status, or religion/belief.
  • Document progress in daily records, observing, recording, and reporting changes.
Training & Background Checks

An enhanced DBS check and full training will be provided before commencing work. Ongoing support from the clinical lead nurse will be available.
