We are seeking an enthusiastic and highly‑motivated Team Lead to join our Dementia Outreach Team and become part of a service that really makes a difference to patients experiencing a dementia‑related crisis, and their families.
As the Team Lead, you will be responsible for providing clinical and managerial leadership with mentoring and educational input to the Dementia Outreach Team multidisciplinary team. This includes acting as a clinical leader and expert practitioner liaising, guiding and advising the multidisciplinary team in the provision of optimum patient experience for people accessing support and assessment. They will also be responsible for supporting the effective role modelling, ongoing management and clinical supervision, and mentorship to the team.
You will be responsible for ensuring that the highest quality care is carried out in accordance with policies and procedures Hampshire and Isle of Wight NHS Foundation Trust.
You will ideally have experience of working with people with dementia and one of the professional registrations outlined within the attached Job Description and Person Specification. The ability to work in partnership with carers/family members and services in the community is essential.
The post holder will be responsible for managing their team by providing mentoring, supervision and education in relation to all managerial, professional and performance processes. Within the leadership role they will be responsible for supporting the team and providing feedback on the evaluation of good and poor practice, identifying training and education opportunities to improve practice and delivery.
The post holder will be expected to work in line with the Trust values and behave in a manner that facilitates the development of autonomous teams and individuals that are empowered, engaged and committed to continually delivering excellent care with compassion. The post holder will be an advocate for supporting dementia care.
They will also be expected to play a proactive role in quality and service improvement and working closely with the Service Manager and the multidisciplinary team, leading auditing of clinical standards of care. Resourcing and supporting on‑going training, education and support to the multidisciplinary team in relation to mental health and learning disabilities to ensure that people accessing the service receive equitable support and care whilst improving patient outcomes.
The post holder will also have responsibility for the recruitment and retention of staff and the budget allocated to the service.
