To work as part of a multidisciplinary team within the GP surgery,providing expert clinical pharmacy services to improve patient care andoptimise medicines use. The post-holder will be an Independent Prescriber andwill have completed the CPPE Clinical Pharmacist Pathway as a minimum. MedicinesOptimisation Conduct structured medication reviews for polypharmacy and high-risk patients. Implement national and local prescribing guidelines and quality improvement initiatives. Identify and manage medicines-related risks and contribute to reducing avoidable harm

Collaborative Working Work closely with GPs, nurses, and other healthcare professionals to deliver integrated care. Participate in multidisciplinary meetings and contribute to clinical decision-making

Patient Education & Support Provide advice on medicines use, adherence, and lifestyle interventions. Support self-management and shared decision-making with patients
