Thepost holder will provide primary support to general practice staff with regardsto Structured Medication Reviews for priority groups, as stipulated by the PCNDES, aswell as other priority chosen pieces of work such as Lipid Optimisationand Drug Monitoring as required.
Main duties of the job
- See (where appropriate) patients with single or multiple medicalproblems where medicine optimisation is required (e.g. COPD, asthma). Reviewthe ongoing need for each medicine, a review of monitoring needs and anopportunity to support patients with their medicines taking ensuring they getthe best use of their medicines (i.e. medicines optimisation). Make, and enactwhere appropriate, recommendations to clinicians for medicine improvement.
- Undertakeclinical structured medication reviews with patients. Make, and enact whereappropriate, recommendations to clinicians on prescribing and monitoring
- DeliverStructured Medication Reviews for the below cohorts of patients in line withDES specifications.
- Provide support and supervision to allocated Pharmacy Technicianswhere appropriate.
- Contribute pharmaceutical advice for the development andimplementation of new services that have medicinal components (e.g. advice ontreatment pathways and patient information leaflets).
- Analyse, interpret, and present medicines data and audits tohighlight issues and risks to support decision making.
- Implement changes to medicines that result from MHRA alerts,product withdrawal and other local and national guidance
- Work with the general practice team to ensure the practices in thePCN are compliant with CQC standards where medicines are involved.
- Support public health campaigns. To provide specialist knowledgeon all public health programmes available to the public.

Person Specification
Knowledge and Skills
- Understanding of evidence-based practice
- Proven ability to evaluate the safety and effectiveness of own clinical practice
- Good understanding of Primary Care Structure and General Practice
- Knowledge of national standards that inform practice (e.g., National Service Frameworks, NICE guidelines etc)
- Excellent interpersonal, verbal and written communication skills
- Time management and ability to prioritise workload.
- Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework
- Use of Microsoft Office (PowerPoint, Excel etc)
Qualifications
- Registered with the General Pharmaceutical Council
- Member of the Royal Pharmaceutical Society of Great Britain or UKCPA
- Postgraduate Clinical Qualification (MSc or Diploma)
- Teaching / Mentoring Experience and / or qualification
- Hold or be working towards an Independent Prescribing Qualification
Experience
- Minimum 2 years' experience as a qualified Pharmacist, demonstrated within a Practice portfolio
- Has experience and an awareness of common acute and long-term conditions that are likely to be seen in General Practice
- Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to GPs when appropriate
- Able to follow legal, ethical, professional and organisational policies/procedure and codes of conduct.
- Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines
- Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge
- Proven ability to evaluate the safety and effectiveness of their own clinical practice.
