Job summary

A local Primary Care Network in Leeds is seeking a clinical pharmacist to enhance medicines optimisation services within its practices. The role involves conducting medication reviews, recommending prescription improvements, and working closely with patients and healthcare teams to improve care quality. Candidates must have 2+ years of experience as a pharmacist, hold a pharmacy degree, and be registered with the General Pharmaceutical Council. This position is full-time and offers a vibrant work environment in a multi-disciplinary team.

Qualifications

  • Minimum of two years experience as a pharmacist.
  • Demonstrates ability to integrate general practice with community pharmacy teams.
  • Experience of working in primary care and in a GP practice.

Responsibilities

  • Conduct face-to-face and telephone medication reviews of patients.
  • Make recommendations for medicine optimisation to GPs.
  • Manage repeat prescribing processes and analyse prescribing data.

Skills

Excellent written and verbal communication skills
Experience in conducting clinical medication reviews
Ability to influence others
Knowledge of medicines optimisation

Education

Completion of an undergraduate degree in pharmacy
Holds an independent prescribing qualification
Clinical Diploma or equivalent
