• Coordinate and deliver comprehensive care to individuals with complex mental health conditions within the community.
• Conduct assessments and liaise with multidisciplinary teams to develop effective care plans.
• Provide expert guidance and support to service users and their families, empowering them to achieve their recovery goals.
• Engage in collaborative working with other healthcare professionals and services, enhancing the quality of care provided.
• Stay updated with evidence-based interventions, ensuring the delivery of the highest standard of mental health care.

• Be a Registered Nurse (Mental Health) or hold professional registration in Social Work or Occupational Therapy.
• Demonstrate evidence of ongoing professional development to maintain up-to-date knowledge in mental health.
• Have experience in community mental health, such as involvement with Home Treatment Teams or Community Mental Health Teams, or experience within acute inpatient settings.
• Possess knowledge of evidence-based interventions for complex mental health conditions, enabling you to provide high-quality care.
