A local healthcare practice in Stafford is seeking an experienced Advanced Nurse Practitioner. The role involves assessing and treating patients, providing clinical leadership, and supporting a collaborative healthcare team. Candidates should be independent prescribers with at least 5 years of experience in primary care. This permanent position offers 5 hours per week, with salary dependent on experience.
Mill Bank Surgery is looking for an experienced Advanced Nurse Practitioner to join ourbusy, well established and high achieving town centre practice, working alongsideour existing supportive friendly clinical and administrative teams.
The position is permanent, offering (5 hours per week) days to be agreed.
Salary dependant on experience.
You will be an Independent Prescriber with experience in assessing, diagnosing and implementing a range of care interventions for patients.
Our clinical team consists of 3 GP partners, 3 salaried GPs, 2 advanced nurse practitioners, 2 practice nurses, and 1 healthcare assistants. We also have PCN additional roles including Clinical Pharmacists, 2 Pharmacy Technicians, 3 First Contact Physiotherapists, a Social Prescriber and a Mental Health Practitioner. We are supported by a resilient and longstanding administrative team.
The practice has a list size of 10,500 patients and the post holder will already be an independent prescriber, required to work as part of our clinical team on a daily basis providing assessment, diagnosis and treatment at first point of contact in the practice, in patients homes, or in a Care Home setting. JOB SUMMARY
To practice as an Advanced NursePractitioner undertaking the assessment, evaluation and planning of patientcare, providing clinical treatments for patients in a primary care setting.
KEY RESPONSIBILITIES
Clinical
Providingassessment, diagnosis and treatment at first point of contact by attending topatients according to patients needs either in the Practice, Care Home settingor patients homes.
Workingin the Same Day Team diagnosing and treating patients with minor illness andother urgent presentations face-to-face or via telephone.
Accuratelytriaging and prioritising patients, including in acute situations,demonstrating the use of a variety of techniques to elicit the history of anevent/illness, including past medical and drug history.
Evaluatingclinical information from examination and history taking, initiating appropriatetreatment and/or referral and requesting and interpreting tests eg., bloodtests, imaging and ECGs to aid the diagnosis of complex conditions.
Usingbroad clinical guidelines (NICE etc) relating them to individual cases,providing assistance and feedback to the organisation and others on how theseshould be interpreted.
Signpostingappropriately to other services in the identified healthcare pathway.
Prescribingmedications as an independent prescriber.
Referringpatients to appropriate care settings, within locally agreed healthcarepathways.
Maintainingaccurate and systematic documentation of patient interactions and clinical careprovided, including adverse events.
Advisingand supporting members of the public on healthy lifestyles, health promotionand disease prevention.
Providingclinical advice to other members of the team where appropriate.
Communication
Liaisingand interacting with multi-professional teams across organisational boundaries,including primary care, social services and secondary care providers accordingto patient needs and appropriate care pathways.
Maintainingcommunication with all healthcare professionals and outside agencies to ensureseamless, patient-centred service provision.
Ensuringeffective communication so that the patient receives the appropriate level ofcare and wherever possible avoiding unnecessary hospital admission.
Takingan active role in maintaining expert communication with all patients, carersand relatives which meets their individual needs.
Dealingwith complaints in a calm and courteous manner, ensuring that, whereverpossible, complaints are dealt with efficiently and satisfactorily at a locallevel.
Communicatingeffectively in demanding and challenging situations, where there may also behostility
Professional
Inputtingand extracting information from the EMIS clinical IT system.
Workingcollaboratively with other members of the health care team such as the DistrictNursing Team, Mental Health Services, staff in care homes, Ambulance Service,Secondary Care etc.
Ensuringthat the wellbeing of patients and staff is maintained in accordance withPractice policies.
Undertakingclinical audits in line with the organisations clinical audit plan, and toparticipating as required in equipment testing and other research.
Contributingto the analysis of information to inform future development of the service.
Participatingand contributing to clinical meetings
Identifyingrisks to service provision, and highlighting them to the Practice Partners
Workingwithin the requirements of Data Protection legislation as it applies to patientrecords.
Maintainingcurrent professional registration.
