An exciting opportunity has arisen for an enthusiastic and motivated Occupational Therapist to join the Admission Avoidance and Discharge Service (AADS) at ELFT. The role involves working in various clinical areas across the wider pathways of the service including Rapid Response, Intermediate Care, Falls Pick-Up Service, and the Admission Avoidance Team (based in ED, Royal London Hospital). The specialist role is based in the community in the borough of Tower Hamlets, with a multidisciplinary team that works across the community and hospital to facilitate early discharge and prevent unnecessary hospital admission. The service uses a discharge-to-assess model with an integrated care model including Reablement and Adult Social Care.
We are looking for a versatile Occupational Therapist to join our team, working with patients across a variety of clinical areas. Applicants should have strong clinical skills, the ability to manage complex cases independently, and provide comprehensive therapy assessment and intervention with evidence-based clinical reasoning.
You will need to be a flexible, adaptable team player ready to work across a 7-day rota.
The post holder will work with partners across health and social care to ensure a joined-up, integrated provision of services. Staff are expected to work across community and acute services to deliver holistic care.
Identify patients at risk of repeated admission and implement measures to minimise deterioration and the need for secondary care.
The Admission Avoidance and Discharge Service includes an Admission Avoidance Team in the Emergency Department, in-reach nurses, Rapid Response, and an Intermediate Care Team using a discharge-to-assess model. The main aim is to reduce hospital stay and admissions by supporting clients in their own homes.
