Overview

Senior Practitioner Transfer of Care Hub (Hospital) – Hackney, UK. £32 per hour. Hourly; Ongoing contract. On-site presence required a minimum of two days per week.

What you will do
  • Lead the Adult Social Care (ASC) representation within the Transfer of Care Hub
  • Coordinate and oversee patient-discharge processes to ensure timely and safe transitions
  • Collaborate with healthcare professionals to improve discharge planning and processes
  • Assess patients’ social care needs and develop care plans that address these
  • Support and mentor less experienced social workers and healthcare staff
  • Maintain accurate and timely records of all cases, ensuring compliance with relevant legislation and regulations
Requirements
  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience as a qualified Social Worker in UK Social Services settings
