What you will do
  • Complete social care needs assessments to identify and address the needs of adults within the community
  • Develop support plans by working closely with individuals and their families to ensure appropriate and tailored care
  • Commission services and resources to meet identified needs effectively
  • Conduct risk assessments and manage risks by attending MARAM meetings and employing safeguarding practices
  • Engage in Mental Capacity Assessments, Best Interest Decisions, Court of Protection applications and Deprivation of Liberty Safeguards procedures
  • Contribute to Continuing Healthcare and Decision Support Tool assessments to support individuals with complex health needs
Requirements of the Social Worker
  • Hold a degree or equivalent qualification in social work
  • Maintain current registration with Social Work England
  • Possess significant frontline experience in UK Social Services as a qualified Social Worker
  • Be a car driver, essential for visiting clients and attending meetings within the community
