Social Worker – Adults ARCH (Active, recovery, community, home) within Hospital Discharge Team in Stockport, £35 Hourly.

Join the esteemed ARCH Team in Stockport for an exciting opportunity to boost your career! We are seeking dynamic and enthusiastic social workers for immediate start on a three-month contract. You will work full-time, five days in the office, within a supportive team dedicated to improving the hospital discharge experience. Essential experience includes hospital and D2A pathway work, and candidates must be DBS checked and registered with SWE.

• Collaborate within a multi-disciplinary team to manage complex cases and hospital discharges.
• Conduct Care Act assessments, mental capacity assessments, and safeguarding work.
• Champion a positive patient journey, ensuring the most favorable outcomes and supporting individuals to live “Gloriously Ordinary” lives.
• Quickly respond to hospital pressures, ensuring patients remain central to decision-making, and uphold the Home First values.
• Drive and support the goals of Stockport Social Care, and participate in change management and statutory compliance.

For this role, you must be a qualified Social Worker registered with Social Work England, with relevant experience in MDT settings, Care Act knowledge, and safeguarding practices.
