As part of the Specialist Palliative Care Team, provide a high quality service within KGH to all patients with specialist palliative care, end of life care needs & end of life discharge needs, irrespective of diagnosis. To provide education, specialist advice, and support to professionals, patients and carers and work with others within the acute setting to provide strategic direction for end of life care.

• The post holder will be supported by experienced nurses to enable them to achieve core skills in agreed areas of specialist palliative care practice through exposure, training and education to agreed levels of competence, to enable autonomous practice within the specialty.
• Alongside the Palliative Care Practice Educator, develop the practice of those working with palliative care and end of life patients within the acute setting.
• Holistically assess patients who have been recognised as dying.
• Assist with assessment of other palliative patients who may have complex physical and emotional needs.
• Alongside the End-of-Life Discharge nurse, work proactively to ensure patients can be discharged to the place of their choice where possible.
• Assist with arranging packages of care for patients at the end of life, tailored to the needs of individual patients and clinical need.
• Be required to work on their own initiative, making decisions to achieve the best outcome for the patient.
• Support the Specialist Palliative Care Nurse in offering expertise in palliative care, symptom control and psychological support of patients and families with life-limiting illness.
• Autonomously, develop plans to meet the complex needs of end of life patients in every acute clinical area.
• Implement and evaluate evidence-based practice within the field of palliative care & end of life care nursing to improve standards of care.
• Participate in the development & monitor the implementation of policies, standards and guidelines.
• Act as a role model through provision of high standards of professional practice and promote a clinical environment that motivates others and supports positive collaborative working.
• Initiate and participate in auditing standards of end of life care.
