A healthcare provider is seeking a motivated and passionate registered Nurse for their 0-19 Service in Sunderland. The role involves promoting the physical, emotional, and social development of children while collaborating with families and partner agencies. Candidates should have a Registered Nursing Degree and a strong commitment to public health interventions.
HDFT are seeking to recruit a motivated and passionate registered Nurse to be part of the innovative 0-19 Service within the HDFT footprint, based within Westmorland and Furness. The successful candidate will work with parents, carers, children and partner agencies to promote the physical, emotional and social development of children.
Contributing to the delivery of the Healthy Child programme by undertaking some mandated contacts and targeted interventions as directed/delegated by the Specialist Community Public Health Nurse (SCPHN) and working in conjunction with the SCPHN to co-ordinate, plan, implement and evaluate a programme of care for children, young people and families.
They will support the completion of comprehensive health needs assessment of clients, families and community and facilitate the mobilisation of appropriate resources to meet identified needs.
The post holder will ensure inter-agency/ professional partnership working between services, statutory and voluntary organisations to facilitate integrated approaches in meeting the needs of children.

The post holder works as a member of the 0-19 Community Public Health Team, being responsible for the assessment, planning, implementation and evaluation of a comprehensive service which meets the needs of the 0-19 population.
The post holder will participate in delivering Public Health intervention and support to children and families in Sunderland.
