This position is responsible for providing Community Living Supports for children enrolled in the YES Waiver. This position will work in full partnership with team members to help the youth meet goals identified in the YES Waiver family team meetings. Community Living Support Provider is a mental health professional/skills trainer that facilitates the modelling, coaching and social skill building in the individual’s community with children 3-18 in Youth Empowerment Services. CLS services are used to increase or maintain personal self-sufficiency, helping an individual to reach his/her goals of community inclusion and participation, independence, or productivity. CLS services may be provided in the individual’s residence or in community settings (including, but not limited to, libraries, city pools, camps, etc.)

• Work in partnership with team members to help the youth meet goals identified in the YES Waiver family team meetings.
• Provide Community Living Support (CLS) to facilitate modelling, coaching and social skill building in the youth’s community.
• Deliver CLS services to children aged 3-18 in Youth Empowerment Services to increase or maintain personal self-sufficiency and support goals of community inclusion, participation, independence, or productivity.
• Provide CLS in the individual’s residence or in community settings (e.g., libraries, city pools, camps, etc.).
• Deliver care in a manner consistent with strength-based, family-driven, youth-guided, and culturally competent values.
• Offer consultation and education to providers regarding the values and principles of the model; monitor progress toward treatment goals; and gather and record necessary data for evaluation.
• Demonstrate aptitude for working with individuals with diagnoses such as bipolar disorder, major depressive disorder, schizophrenia, schizoaffective disorder, and other psychotic disorders; apply knowledge of psychotherapeutic medications and evidence-based practices and contraindications in recovery promotion.
• Apply knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis, and wellness and recovery-oriented systems of care.

Salary: $27.19 – $28.94/hr

• Minimum Qualifications: A Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field. Knowledge of mental illness as defined in the DSM-IV is required. Must have a valid Texas driver’s license.
• Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred.

Salary Amount: $27.19 - 28.94/hr
