Job summary

A healthcare services provider in Ontario is seeking Nurse Practitioners and Physician Assistants for a flexible PRN role conducting in-home Health Risk Assessments for Medicare Advantage members. Qualifications

• Active NP or PA license required, excluding Pediatrics, PMHNP and Women’s Health specialties.
• Minimum availability of five full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of your home.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review each patient’s medical history, medications, lifestyle, and social factors.
• Conduct basic physical and mental health evaluations.
