A community health service provider in Chertsey is seeking a Community Healthcare Assistant to support children, young people, and families across various settings. We are seeking a compassionateCommunity Healthcare Assistantto join our Healthy Family Team, supporting children, young people, and their families across homes, schools, early years settings, clinics, and the wider community.
The role involves assisting in the delivery of universal, targeted, and specialist services, providing high-quality, evidence-based care to improve health outcomes. Working under the supervision of qualified professionals, the post holder will carry out assigned tasks and may adapt care packages based on individual progress and needs.
Base
Our Healthy Family Team supports our Surrey Children and Family Health service and you will be based at one of our Office Hubs across Surrey with the opportunity for hybrid working.

Deliver care and support to children, young people, and families across homes, schools, early years settings, clinics, and community venues, under the supervision of qualified professionals.
Assist with assessment, review, and progress monitoring using structured tools and feedback to qualified staff.
Support universal, targeted, and specialist programmes, including the Healthy Child Programme, health promotion initiatives, and group interventions.
Run, coordinate, and support training, workshops, and group sessions for children, families, and professionals.
Build and maintain effective relationships with families, schools, and multidisciplinary teams; provide guidance and support as needed.
Maintain accurate, contemporaneous records and documentation in line with organisational and national standards; support administrative tasks.
Follow safeguarding procedures, identify risks, elevate concerns, and participate in safeguarding supervision.
Use clinical reasoning to implement interventions, make referrals, and adapt care packages to individual needs and outcomes.
Ensure safe use, storage, and transportation of equipment and resources; contribute to service audits, quality improvement, and policy development.
Participate in professional development, supervision, appraisal, and training of students, junior staff, and new team members.
Work flexibly across services and settings, manage own caseload, and adhere to organisational policies, procedures, and lone working guidelines.
