This is an excellent opportunity for a dedicated and empathetic Registered Nurse to join the Outpatient Services team at St Lukes Hospice. These procedures include venepuncture, blood product transfusions, drainage, CVC line care and IV drug administration. Training will be provided if competencies in relevant activity not obtained. He/she will participate in teaching of other staff, as appropriate, including students.He/she will be required to actively contribute to the development and well-being of colleagues

Job responsibilities

QUALIFICATIONS:

The Post holder is required to have a Registered NurseQualification, an appropriate recordable post basic Certificate/Statement ofCompetence, or equivalent level of skill acquired through experience.

The Post holder is preferred to have a minimum of oneyear post qualification of basic experience or an interest in Palliative Carefrom qualification. Consideration will give to applicants who do not fulfilthese criteria based on individual employment history, experience andcompetence.

The selected person should have validated completionof St. Lukes Qualified Nurse Competencies and Syringe Driver Competency and forthose newly employed on the Hospice Scale 14 the St. Lukes competencies willneed to be achieved within the six-month induction period.

MAIN DUTIES:

The Post holder will participate as a member of theclinical nursing team in the maintenance and development of agreedstandards. RESPONSIBILITES:

1. Clinical

1.1 Ensure the physical, spiritual, emotional,psychological, social and cultural needs of patients are identified andattended to, providing and maintaining the highest possible level of nursingcare.

1.2 Devising individualised care plans andongoing reviews to ensure care plans are individual/appropriate for patients,encouraging discussion and participation by all team members includingthe Multi-Disciplinary Team giving and receiving patient-centred handovers.

1.3 Active involvement within the Multi-Disciplinary TeamMeetings. Ensuring patients are identified and presented for review in a clearand concise manner

1.4 As part of patient reviews, identify and providesupport to formal carers, relatives and friends of patients/residents,maintaining confidentiality at all times and ensuring good relationships withpatients relatives by providing frequent contact and availability. Liaisingwith support services.

1.5 Ensure that all medication, treatments, therapies andcare interventions are clinically reviewed and administered in accordance withHospice Policies, Procedures and Standards.

1.6 To ensure the safe custody, storage and control andadministration of all drugs in line with national standards and the HospiceMedicines Management Policy maintaining awareness of the Medicines Managementmeeting and role of the Accountable Officer for Medicines Management.

1.7 To work closely with all members of St.Lukes Hospice Community Services to ensure patients are supported effectively,and unwanted and inappropriate admissions to acute care are avoided.

1.8 Towork effectively with patients and carers from diverse cultural and socialbackgrounds.

1.9 Ensure that verbal and writtencommunication is provided to a high standard and that

documentation is completed accurately on SystmOne electronic records.Sensitive information must

bedocumented in patients paper records. To be pro-active in checking standardsrelated to record

keeping.

1.10 To ensure that effective communication is maintainedbetween staff, patients and their relatives so that appropriate support isprovided through illness and death. Maintaining coordination of patient is akey function of the St. Lukes Hospice.

1.11 Take responsibility for risk management and assessmentwithin the work place, taking all steps possible to safeguard the health,welfare and safety of all staff, patients and visitors.

1.12 Should be providing, or working towards providing, aspecialist level of practice in all areas of palliative care provision.

1.13 Ensure Hospice policies are adhered to atall time with relevant clinical policies including CVC line care, safeadministration of blood products, venepuncture and cannulation

1.14 Undertake holistic needs assessments ofpatients identifying physical, mental and social needs and referring asappropriate, to the Hospice and community MDTs.

2 Education/Research/Development

2.1 Attendance at In-House Study Days, Staff meetings,along with any meetings relevant to associatedlink role is compulsory.

2.2 Completion oforganisational competencies within the first 6 months.

2.3 Participate as required in programmes of educationprovided by the Hospice. Staff will berequiredto cascade learning back to remainder of nursing staff following attendance atexternal Study Days.

2.4 To act asfacilitator for all new members of staff, students and work experience onclinical placementsas required.

2.5 To work in partnership with other teams ineducational audits and triennial reviews

2.6 To be involved in formal teaching internally andexternally as required. Having an active role in promotion of hospice servicesi.e. Community Engagement

2.7 Ensure the practical instruction and supervision ofnurses in training allocated to the clinical area

with agreed learning outcomes and currentUnit teaching programmes.

2.8 To maintain awareness of relevant up todate research, in order to practice and teach research and evidence basednursing.

2.9 To maintain professional development andawareness and keep up to date with changing clinical practices. In line withRevalidation Guidance.

2.10 To actively use reflection in his/her ownpractice, encourage it in other team members and participate in mandatoryclinical supervision.

2.11 To actively participate in team debriefs to reflect onsituations and practice, cascade learning and share experiences.

2.12 To undertake extended roles in the Unitfollowing recognised training as appropriate (e.g. Link Nurse Role). Includingdelivering training and ongoing support to other staff across the organisationin relation to the role.

2.13 To assist with audit activity within the Clinicalareas and Hospice in general as part of the organisations annual audit plan.

3 Managerial/Organisational

3.1 To take a lead position (as the nurse incharge of the shift) as required in the day-to-day running of the ClinicalOutpatients

3.2 To demonstrate and maintain good management andorganisational skills.

3.3 To ensure that both verbal and written communicationis provided to a high standard and that

documentation and electronic records arecompleted accurately and according to policy. To take a lead role in monitoringand improving these where necessary.

3.4 To assist and supervise other members of the nursingteam to ensure all staff work to achieve an acceptable safe standard and highquality of care.

3.5 To report and investigate complaints/untowardincidents and complete relevant

documentation

3.6 To exercise sound leadership and clinicaldecision-making skills at all times and facilitate the development of same inother staff members.

3.7 To participate in the development and on-going reviewof Policies, Procedures and Standards as required and implementation of samewithin the relevant Hospice Services.

3.8 To contribute to the strategic planning for thedevelopment of staff and the service provided

within the relevant Hospice services.

4 General:

4.1 In the absence of Senior staff to takecharge as required and liaise with Senior staff on call as necessary.

4.2 The Post holder must acquire and maintainan up-to-date working knowledge of all Hospice Strategies, Policies, Proceduresand standards.

4.3 Ensure Health & Safety regulations are adhered to.

4.4 Assist in ensuring the economical use and ordering ofmaterials and equipment, maintaining an awareness of the need for goodhousekeeping and helping to ensure expenditure is within budgetary limits.

4.5 To maintain strict confidentiality within the Hospiceand ensure that all staff and volunteers under their management adhere to theHospice Confidentiality Policy.

4.6 Each nurse after 6-12 months, is expected to take on aspecial interest or Link responsibility, in which they will be expected to actas an information resource to the rest of the team, and provide practiceevidence based in that area. This will include informal and formal teaching andmay include community engagement.

4.7 To undertake any other duties which are considered tobe consistent and relevant tothe post.
